Become a New Patient

We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:

1. Make an Appointment
2. Sign up for our patient portal
3. Download your patient forms online through the patient portal

When you come to our office for the first time as a new patient, we’ll ask you to complete some initial forms, 包括一个 Authorization and Consent for Treatment form，如果您无法在预约前从患者门户网站下载它们.

What to Expect at Your First Appointment

You’ve made your appointment to get established as a new patient. 现在?

Other patient information to bring includes:

    • Copies of medical records from previous doctors
    • History of surgical procedures with dates
    • Any current or recent test results or x-rays

When you enter the office, please sign in at the receptionist desk. Provide the staff member your completed paperwork. There may be other forms to sign at that time.

Common Courtesy is Your Expectation and Our Policy
